Why the hell was the D Man from Duke who tried to do an Andre the Giant headbutt on Chris Kavanaugh not ejected. Not in the rules? Lack of replay to see how bad it was? General ref incompetence?

There is a 3 minute penalty and ejection for egregious contact to the head - that play was the definition
Good to know it's in the rules.

I know two minutes non releasable is a big penalty, but that one deserved the maximum of an ejection.
