Never heard of such a thing. If the team rule coincides with a criminal statute or a major DuLac rule, sure. But a provision only of the team rules? Never seen that.

It could be a serious offense, but it could be one too many in a series of not altogether egregious issues. Sometimes enough is enough.

I have nothing to back this up, but I always translate that in my mind as substance violation.
