HIPAA (Health Insurance Portability and Accountability Act) only applies to healthcare providers and, in most cases, athletes sign a waiver that allows the provider to share their medical information with the organization for which they compete, professional or educational.

If the medical information were personal in nature (vaccine status, mental illness, unrelated disease or illness for example) I think the institution would be ethically, but not legally, required to respect the athlete's wishes for privacy.
